1. You can file a report either by phone (1-888-223-2022) or fax (1-877-511-2141)

2. Be specific about the location. Ideally, include the following information in your report:
   • Street address of closest building
   • Landmarks, such as 'two poles south of the corner of Highwood Avenue and Hauxhurst Avenue'
   • Cross streets
   • If you can find the pole number on the problem light, include it

3. State the problem with the light clearly:
   • Light is out
   • Light keeps turning off and on
   • Light stays on
   • Pole is leaning, broken or down
   • Globe broken, access door missing, exposed wires

By providing PSEG with as much information as possible, you'll insure a speedy repair of the light.
